| Our Home and Community Care (HACC) programs are supported by the Department of Ageing Disability and Home Care (DADHC). Our programs assist individuals in residing at home with a sense of comfort and safety and help prevent premature admission into residential care. The social activities focus on friendship and companionship. Our NESB Dementia Monitoring program provides culturally and linguistically appropriate service to Greek, Arabic, Macedonian, Chinese, Italian and Spanish speaking clients living in the St George LGA. |

Our aim is to provide Dementia Monitoring and Support for frail older people in the local government areas of Hurstville, Kogarah and Rockdale. This service also provides meal monitoring where required. Our NESB Dementia Monitoring program provides culturally and linguistically appropriate service to Greek, Arabic, Macedonian, Chinese, Italian and Spanish speaking clients living in the St George LGA.
Dementia Monitoring refers to services which monitor the health and well being of people with dementia and which offer social support to those people. Our service will also provide feedback to case managers about other support services being provided for individual clients. |

| Access to Service |

| How we assess clients for Aged Care Programs: A Coordinator will visit you and/or your carer or advocate in your own home. Here we discuss how our service can best assist you with any problems you are experiencing. A bilingual worker will also visit with the coordinator if translation is required. The service you receive will be determined by how much support you or your carer requires in areas such as: |
| Tasks of daily living and/or Social support |
| After the coordinator has visited, we will work out a Client Care Plan for how visitors can best assist you. This plan will be based on what you tell us is causing you difficulties and what our visitors can provide. If another agency can provide a particular service, we will,with your permission, refer you to that agency. |

| Our Not for Profit service relies on financial assistance from our members, clients, carers and sponsors. However, people with need for our supports will not be disadvantaged or denied access to services. Clients can discuss any concerns with their coordinator if you feel you are not in a position to pay. |

| FEES: A contribution for service is charged per visit. |

| Client Advocacy |
| |
| Clients are entitled to appoint an Advocate to represent their interests to our Aged Care Programs. An Advocate may be a family member, friend or carer who is trusted by you to speak on your behalf. |

| An advocate is a support person who can help you have your needs met. If you would like to have an advocate, and you don't have one; you can contact |
| |
| Advocates will be accepted by Kogarah Community Services Inc. as representing your interests. |
| |
| Advocates may be used during assessments, complaints or for any other communication between you and Kogarah Community Services Inc. representatives. |
| |
| The Coordinator and relevant employees of Kogarah Community Services Inc. will inform you of your right to use an advocate and will regularly remind you of this option. |

| If you feel the assigned Home Visitor is not suitable with meeting your needs, a change of visitor can be arranged. Contact the program coordinator for making changes. |

| Visitor Illness or on Leave: |
| If your Visitor is sick or on holidays, we will where possible replace them with another Visitor. We will advise you as soon as possible of any change. |

| Neighbour Aid Program |
| |
| The Neighbour Aid program gives opportunities for seniors within our community to participate in regular group activities. |
| |
| This social support is provided to frail aged clients living in the Kogarah Local Government Area only. Our Bus outings cater to seniors within our community who are actively interested in going out more and meeting new people. |
| |
| Trips are held most weeks and consist of visits to local parks, reserves and places of interest. Morning Tea refreshments are provided. |
| |
| Cost range from $6.00 includes pick up and drop off to your door or at an agreed location. Costs vary depending on outing. |
| |
