To complete your enrolment you will need:
- CRN’s – if you have not applied for the Child Care Subsidy (CCS) yet you can add the CRN’s at a later date. Please note that the child and parent CRN must be different
- soft copies of any allergy/asthma action plans, court orders or parenting agreements.
- the details of an additional emergency contact other than parents/guardians
- the details of your local GP/medical centre.
- Copy of your child’s immunisation statement
NOTE – Occasionally there is an issue when parents enter their direct debit details on the enrolment form and save – an error message will appear – please do not re-enter details they have been received – if you have any concerns please contact the office on 9553 6506 (ext. 2)
